
Carrier ID cards
COBRA is a continuation of your existing coverage, so can usually use your existing ID card for your COBRA coverage. Typically, carriers will not issue you a new card, so be sure to hang on to your ID card so you can use the same group number and member ID numbers associated with your coverage through your former employer.
In the event you’ve misplaced your ID card, that’s okay! The easiest way to get a new ID card is to login to the carrier’s online portal or call into their service center and request a new one. You can usually request a digital one to use in the meantime while you wait for a hard copy to come in the mail.
Most dental and vision providers just require a social security number and date of birth to look you up in the carriers' systems, so just have that information handy if you go to an appointment for yourself and your dependents.
